ChangePoint Obesity Prevention and Weight Management Service

ChangePoint Obesity Prevention and Weight Management Service provided by Everyone Health, funded by Nottinghamshire County Council.

Everyone Health no longer take referrals for Tier 3 children and adult patients (BMI ≥ 98th centile and over 35 who are not suitable for Tier 2, respectively).  The contract for this service across Nottinghamshire ended in March 2018. Please see Bariatric Surgery (Tier 3)
 
Everyone Health Tier 1 (universal & targeted prevention) and 2 (healthy lifestyle weight management) services, commissioned by Nottinghamshire County Council, are still available.
 
Tier 2  criteria is as follows:
 

Adult Weight Management Tier 2 Criteria (Including enhanced Tier 2 Adult Weight Management):

  • BMI 25  – 30 with high waist circumference
  • BMI 30 – 49.9
  • BMI 30  –  44.9 with co-morbidities
  • BMI 27.5 – 47.4 for South Asian, Chinese, Black African, African Caribbean population groups
  • BMI ≥23 for South Asian, Chinese, Black African, African Caribbean population groups with high waist circumference

*High Waist Circumference Men ≥102cm, Women ≥88cm


Patients who would not be accepted onto the Tier 2 service as they require Tier 3 Adult Weight Management are:

Child Weight Management | We have a new programme for Child Weight Management, under the name of ‘Alive N Kicking’ – this will replace Jump Avenue and Jump Academy. The programme will run in each district for children aged 5-17 (split between 5-11 and 12-17). The course will run all year round for children and their families to access, in local communities. There is now no upper limit in BMI for children to access the service, however some children may require assessment on a case by case basis to assess their suitability for the programme, including co-morbidities and appropriateness for group intervention.

Weight Watchers | Please see attached document for Weight Watchers, where patients with a BMI >25 could access 12 weeks free with Everyone Health.

Slimming World | Please see attached document for Slimming World, where patients with a BMI >25 could access 12 weeks free with Everyone Health.

Referral Form | Please see attached documents for the new referral form. This includes information on the new Tier 2 criteria. These are available on Ardens. 

Maternity | Please also see the section of referral form for Maternity patients, who require nutritional support pre and post-natal.

 

Health Walks

Everyone Health have set up FREE health walks across Nottinghamshire, designed to get people more active and to provide an opportunity for the community to come together. The walks are led by friendly, fully qualified walk leaders and last between 30 and 60 minutes.
