Type 2 Diabetes

NEW Self Referral to DESMOND

To increase access to DESMOND (Diabetes Education Self Management for Ongoing and Newly Diagnosed) we now accept Self-referrals from patients.
 
To refer themselves in, the patient will need to know when they were diagnosed (If it is less or more than 12 months is enough) latest HbA1C, Total Cholesterol, LDL Cholesterol, HDL Cholesterol and Blood Pressure.
 
The DESMOND team will be out updating display boards in surgeries and dropping off new leaflets and information packs to all the surgeries in the area over the next few weeks. If you would like your pack earlier, we can ensure you get it sooner. Please see the below leaflet with details patients will need to have when they call or email to refer themselves and an updated poster. Practices can still refer to DESMOND using SystmOne electronic referral this is just an additional way that the team will accept referrals.

 

 

DESMOND systmOne e-referrals

Please ensure the consent to share has been actioned. If this has not been ticked as a “yes” to consent to share then we cannot see the referral information. Please see the attachment to show you how to do this if you are unsure.

Please ensure that the task group DESMOND has been chosen on the referral. We have a number of task groups within the department and if the Task group has not been chosen as DESMOND it comes through unassigned and has the potential to get lost as it comes through as unassigned. Please see the attachment to show you how to do this if you are unsure.
